POSITION ANNOUNCEMENT

Public Health Nurse II – Care Manager

Description:  The Henderson County Department of Public Health is accepting applications for one full-time Public Health Nurse II- Care Manager position in the Community Health Nursing Unit. The primary purpose of this unit is to prevent adverse pregnancy outcomes, improve birth outcomes, promote use of the medical home, link children and families to community resources and provide education and family support for ongoing health care to ensure the highest quality of care. Programs include Care Management for High-Risk Pregnancies (CMHRP) and Care Management for At-Risk Children (CMARC). Groups targeted include pregnant women, infants, and children who are at high risk for medical or developmental disability and other at risk and underserved populations such as uninsured and non-English Speakers. Services are provided in diverse settings including the health department, patient homes, childcare facilities, schools, and the community.

Schedule: The schedule is typically Monday through Friday, 8:00 AM to 4:30 PM, with some extended hours (including weekends) to meet client needs and during public health emergencies.

Knowledge, skills, and abilities:  Considerable knowledge of and skill in the application of nursing theory, practices, principles and techniques employed in the field of public health and related programs; considerable knowledge of and the ability to apply principles and practices of public health; knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health, including health disparities; ability to plan, coordinate and oversee work of others; ability to deal tactfully with others and to exercise good judgement in appraising situations and making decisions; ability to deal tactfully with others and to exercise good judgement in appraising situations and making decisions; ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records, reports and medical instructions; adequate computer skills to allow communication, patient record documentation, and accessing of information.

Preferred Qualifications: Previous Care Management experience. Previous experience in pediatric or obstetrical nursing. Spanish language skills.

 

Required Minimum Training: Graduation from an accredited school of professional nursing and one year of professional nursing experience. A current license to practice as a Registered Nurse in North Carolina by the N.C. Board of Nursing. Individuals employed without a Bachelor of Science degree which includes a Public Health Nursing rotation or a Master’s in Public Health, will be required to complete the “Introduction to Principles and Practices of Public Health Nursing” course within one year of employment as a condition of continued employment in accordance with Health Services Commission Rule.
