Adult Protective Services:
Our APS staff received, investigates, and assesses report of abuse,
neglect and exploitation of elderly and disabled adults. Services are
designed to identify, remedy, and prevent problems that result in
abuse, neglect, or exploitation. The safety and protection of the
"at-risk" adult is our primary consideration while acknowledging their right to self-determination.
Guardianship:
Provides aged/disabled individuals who the court finds are unable to
make responsible decisions regarding their person and/or property and
who have no family willing or able to serve in this capacity. These
services helped assure the health, safety, and well being of thirty-two
individuals.
Representative Payee:
At the request of the Social Security Administration, we provided these
services to six adults who are unable to manage their own resources or
protect their own interests. Interfaith Assistance Ministries also
provides this service.
In Home Services:
Community Social Services Assistants (CSSA) help give elderly and disabled adults the option to stay in their home rather than seek out of home placement by assisting with money management necessary household tasks. A member of our staff usually visits each assigned client's home once a week. An average of fifty individuals a month receive services at this time.
Enhanced Care Case Management:
This is a Medicaid based program provided in adult and family care homes designed to help residents age-in place avoiding a higher level of care. Case management includes assessing in-home service needs, coordination, and making sure services are delivered.
Adult Day Care:
This program is designed to offer care, socialization, and activities
for elderly and disable adults. This service enables families to
maintain employment and other activities. It is also used to provide
brief periods of respite for caregivers.
Long Term Care Placement and Licensure:
We oversee licensing of family care homes and adult care homes. Thirty
homes are monitored monthly to ensure the safety and well being of
residents and home compliance with state rules and regulations. We also
assist families who are making decisions about facility placement.
Services For The Blind:
